Your quality of life matters at TRM Hospice. Hospice care is a special kind of care that focuses on managing pain and symptoms associated with life-limiting illness to improve the patient’s quality of life. TRM Hospice has helped patients and families live their best quality of life since 2017.
TRM Hospice cares for the patient and the family
TRM Hospice cares for the patient and family together. One of the most important things that we hear from families is that we serve them and their loved ones with heart. That’s because we understand how difficult a time it is for everyone when a loved one is on their end of life journey.
We take a team approach to care that includes a physician, nurses, social workers, chaplains, hospice aides, volunteers, and bereavement counselors. We provide hospice service wherever patients and families need us – in their home, an assisted living facility, retirement community, or wherever the patient is located. Our respite care can go into other facilities and provide rest for family and caregivers. We meet patients wherever they are.

TRM Hospice helps patients and families live their best quality of life by providing top rated care that meets their specific medical, emotional, and spiritual needs.
Each patient and family collaborates with an interdisciplinary team that includes a physician, case manager, hospice aide, social worker, chaplain, bereavement specialist, and volunteer.
On-staff physicians are Board Certified in Hospice and Palliative Medicine and specialize in managing pain and symptoms that are unique to end-of-life care.

Originally Accredited by Community Health Accreditation Partner (CHAP) for exceeding quality and compliance standards since 2009 and the National Institute for Jewish Hospice.
Our Personal Care Homes is open to families who are unable to manage their loved ones at their own place of residence and need additional help with ADLs in a safe home away from home. Routine home care services are offered by our hospice care team at the patient’s place of residence.
TRM Hospice care is covered by Medicare, Medicaid and most private insurance plans and charity care is provided for those in need.

Service Standards
Service
Patient care and quality service are our highest priorities and primary reason for existence. We will be the leader in quality care in each community we serve.
Urgency
TRM will respond to referrals immediately, gather patient clinical data to evaluate for hospice eligibility, and if appropriate, admit the patient.
Caring
We will provide compassionate, timely, and effective pain and symptom management to patients and offer support to their families, 24 hours a day.
Responsibility
We recognize the special relationship between attending physicians and their patients and we offer to provide those physicians regular patient status reports on any schedule they request.
Communication
Telephone calls to our staff will always be answered promptly and politely. Trained staff aims to respond to urgent calls made to our after-hours call service within 15 minutes, 24 hours per day.
Response
We will address patient, family, or physician questions, issues or concerns within 24 hours of the appropriate staff being notified about the issue.
Compassion
We will care for our patients and their families with the same compassion, dignity and respect we give our own families.
Feedback
We regularly survey our patients, families, facilities and physicians to ensure we are meeting our service commitment to the people, professionals and communities we serve.

What are the Goals of Hospice Care?
TRM is focused primarily on comfort and pain management for our patients.
Our Key Objectives:
- Keep patients from unnecessary hospital visits – all too often we see patients nearing the end of their lives unnecessarily traveling to and from the hospital, often via traumatizing ambulance rides. These visits are incredibly disrupting for patients, their caregivers and families and frequently result in negative physical outcomes. Instead, our team of clinicians allows patients to be cared for wherever they call home – providing the comfort and pain management needed to weather any physical disruptions.
Engage in frank conversations – Like advance directives, we are firm believers in asking the right questions and knowing how to best achieve the goals.
