Hospice Services
OUR HOSPICE SERVICES INCLUDE:
Nurse visits
Certified nurse aide visits to assist with personal care
Physician and nurse on call 24 hours a day, 7 days a week
Social worker assists with community resources, mobilizing support and counseling
Chaplain provides faith-based spiritual support
Volunteers visit or provide caregiver breaks

AT TRM HOSPICE WE OFFER FOUR LEVELS OF HOSPICE CARE:
- ROUTINE HOME CARE
Hospice care at home, or routine hospice care, is the most common level of care in hospice. A person may choose this level of care when they do not require continuous hospice care. Usually, this means they are not in crisis — they are stable and can control their symptoms. People may receive routine hospice care in their home and not in an inpatient facility. - GENERAL INPATIENT CARE
- Temporary inpatient care in a hospital or care facility, when necessary, to treat pain or other acute symptoms, when routine home care or continuous care is not sufficient
- A team member coordinates care with hospital staff
- Aimed at making the patient comfortable then transitioning back to home setting
- Includes daily visits by a team member to ensure services are provided according to hospice plan of care
- INPATIENT RESPITE CARE
Allows for the patient to be placed in an inpatient setting for up to five days to give family/caregiver relief and prevent caregiver fatigue
Hospice will coordinate care with inpatient team to ensure continuity of care
- CONTINUOUS CARE
- One-on-one nurse and nurse aide care provided in the home setting during brief periods of crisis for pain control and acute symptom management
- Continuous hospice care focuses mainly on continuous nursing care, though a person also may receive homemaker services, hospice aide, or both.
- A patient may receive continuous hospice care in their home and not an inpatient facility.
- Typically, continuous hospice care is available only during short periods of crisis and only as necessary to keep the person at home.
- Someone may require continuous hospice care if their pain or symptoms become uncontrollable.
WHICH LEVELS HAVE MEDICARE COVERAGE?
People with Medicare Part A can receive all four levels of hospice coverage as long as:
they choose a Medicare-certified hospice
a doctor certifies they have a terminal illness with a medical prognosis of 6 months or less
they elect the hospice benefit and waive their rights to Medicare payments
However, they may be responsible for a coinsurance payment, such as :
Medication: The coinsurance for each prescription approximates 5% of the drug’s cost, but it will not exceed $5 per prescription. This includes palliative drugs and biological prescriptions .
Respite care: The coinsurance for each respite care day is equal to 5% of what Medicare pays hospice for that day. It will not exceed the inpatient hospital deductible.
Personal Care Services
PROFESSIONAL & PERSONAL SUPPORT
Personal care is a service that helps families ensure that a loved one has a trusted ally at their side during day-to-day life. A care provider is available to meet the client's social needs regularly, coming for occasional short visits or checking in days depending on the Client’s level of need or preference or staying the whole day. Being able to spend a few hours in the company of a pleasant, friendly, and helpful Care Provider can make an enormous difference for Clients. But, just as important, family members can know that their loved one is not lonely between family visits.
WHAT DO PERSONAL CAREGIVERS DO?
Personal and Home Care Aides provide housekeeping and routine personal care services. They clean the patient's house, do laundry, change bed linens, shop for food, and cook meals. In addition, aides may drive the client to doctor's or other appointments, help them bathe, get dressed, and eat.
COMPASSIONATE & COMPETENT CARE PROVIDERS
Personal Care Services is a solution that empowers Clients to maintain a rich and meaningful social life at any age. Silver-spring Healthcare Services professional Care Providers can assist by:
- Maintaining socialization and communication skills through frequent interaction and friendly dialogue
- Stabilizing during walking by holding a hand or arm in the home or out and about
- Accompanying Clients to appointments, errands, or events
- Providing grooming and cosmetic assistance (brushing hair, applying makeup, filing and polishing nails) to improve self-confidence and self-image
- Performing light household tasks (preparing meals, doing laundry, straightening living areas, etc.) to make the home environment pleasant
COMPANION CARE CAN SAVE A LIFE
An attentive Companion facilitates communication with family and other Care Providers regarding a Client’s overall health and state of mind. Since the Companion interacts with the Client regularly in everyday settings, they may catch warning signs of potentially serious issues before they become crises. For example, a Caregiver may notice changes in behavior or witness unusual incidents that deserve close attention. These insights can be invaluable in ensuring the highest possible level of proactive care.


In Patient Care/Hospice
For families requiring more help with loved ones and who would prefer their loved ones to be in a 24-hour care facility, our private pay personal care homes provide:
- 24/7 General Supervision: Continuous monitoring and management by a dedicated care team.
- Comfortable Facilities: A serene environment designed to provide comfort and tranquility for patients and their families.
- Comprehensive Support: Access to Hospice social workers, chaplains, and volunteers to address emotional and spiritual needs.

Multilingual Care Services
